Tax Exempt Form (pdf) - Tax exempt form is to be used when making ALL purchases for the church.

Expense Reimbursement Form (pdf) - Expense reimbursement form is used when you've made a purchase with your own money and need a reimbursement.  Please attach the receipt to the form and turn in to the appropriate department leader or CAC member.

Expense Recap Form (pdf) - Expense recap form is used when you've made a purchase with a church credit card or check.  Please attach the receipt to the form and turn in to the appropriate department leader or CAC member.

Caregivers Form (pdf) - The Caregivers form is used when you know of someone in the congregation that is going through extreme benevolence situations like (caregiver of the family, going through divorce, death in the family or a long term illness). Fill this out and turn it in to the church office.
