REGISTRATION IS A 3-STEP PROCESS

Step 1: Download Registration Form

Click the icon and follow the steps to register. Fill out the form and download it.

Step 2: Submit the Form Online

Email the completed form to cody@dominionchurch.org 

Step 3: Pay all deposits

Click the Icon to pay online. Please mark it "Lift Retreat". You can also pay through the DC App, or with check or cash.
$50 Deposit is due in order to reserve a spot. The remaining balance will be due before we leave for Lift.

Print and fill out this form to turn in the morning we leave for camp. THIS FORM WILL NOT  BE ACCEPTED UNTIL THE MORNING WE LEAVE FOR CAMP.

This only has to be filled out once a year. You do not have to fill this form out if you have already done so this year.
